Since there are normally 4 parathyroid glans present in the neck and only one is affected in primary hyperparathyroidism, localization of the pathologic gland is paramount to limit the potential for complications by too extensive surgery. While preoperative imaging can identify the pathologic gland in about 90% of cases, experienced parathyroid surgeons will be able to identify the pathologic gland in about 98-99% of cases during surgery. For this reason, a famous American parathyroid surgeon once quipped: “The best localization procedure is to localize an experienced parathyroid surgeon”.
